Unfortunately, I don't know how to reproduce this consistently. It has only happened to me once, on an SSH remote that I use to edit notebooks on a near-daily basis. I was struck by the fact that someone else has reported this before: #128774 which makes me think I'm not crazy.
What I did:
Create a Jupyter Notebook in VS Code.
Write a bunch of code across many cells.
Leave VS Code open for a while (on the order of days).
Return to find your Notebook completely wiped clean with all your work erased. Zero cells will exist in the Notebook.
